Public demonstration only. This page shows the complete consent form and disclosure provided to employees for carrier and Twilio review. It does not submit, save, or transmit any information.
How the web opt-in process works
Step 1. Horizon Roofing provides the employee with the approved web form before enabling operational text messages.
Step 2. The employee enters their name and the mobile number they control. The consent checkbox is unchecked by default.
Step 3. The employee reviews the program description, message types, variable frequency, possible carrier charges, STOP and HELP instructions, and links to the Privacy Notice and SMS Terms.
Step 4. The employee affirmatively checks the consent box and submits the form. Consent is voluntary and is not a condition of employment.
Step 5. Horizon Roofing records the submitted mobile number, date and time, form version, consent language, and source before enabling SMS.
Step 6. An employee can revoke consent at any time by replying STOP or calling Horizon Roofing. A later re-enrollment requires a new affirmative opt-in record.
